Application for use of Willow Oak Facilities 

Please note a "Submit' is required after each signature. 


The Applicant acknowledges receipt of the Rules and Regulations and agrees to adhere to them at all times while using the Willow Oak Hall facilities. The applicant assumes full responsibility for all attendees during their event. Representatives of the Fire District may halt the event or contact the Yolo County Sheriff's Department for issues such as intoxication, serving alcohol to minors, disorderly conduct, property damage, or any violation of rental rules. The applicant agrees to these conditions.

Hold Harmless Agreement (Required)


The Applicant agrees to indemnify and hold harmless the Willow Oak Fire Protection District, its officers, agents, and employees from all losses and expenses, including costs and attorney fees. This indemnification applies to liabilities imposed upon the Willow Oak Fire Protection District for damages, including bodily injury and/or death, sustained by any person or damages to property, including loss of use thereof. These liabilities may arise out of or in consequence of the performance of this agreement, provided such injury or damage is due or claimed to be due to the acts or omissions of the Willow Oak Fire Protection District, its officers, agents, or employees.

Alcohol Provision (Required)


If 'NO', complete the following agreement:

The Applicant agrees that no alcohol whatsoever will be brought onto the premises or served at the function. If any type of alcohol is found on the premises or is seen being consumed by anyone at the event, the event will be terminated immediately, and all attendees will be required to vacate the premises. In addition, all rental fees will be forfeited.
